CGA absolutely wasn't *meant* for composite. IBM was a business machine, CGA was meant for text mode, in which it can happily to 16 colors. Graphics were an afterthought, and at this point the card had no ram left, hence no ability to hold a palette.

If these are Windows, you'd need both Windows and DOS to run them back in the day, as one would load on top of another. DosBox can't run Windows games unless you install Windows 3.x on it first.

Eyes aren't cameras. Things that emit light don't look the same to a human eye. Drawing black lines on a modern screen isn't emulating scanlines.

There's one pretty terrible thing about BloodGDX: it requires Java. I use Java portable launcher to get around that. Oh, and also don't use fullscreen mode, but rather just set it to your desktop resolution with a "borderless" option - much better performance.

Essentially yes. Roland MT-32 and General MIDI aren't the same format, but they are still MIDI devices. The sound cards you've listed had FM synthesis chips. SB16 had an option to add in a MIDI daughter board though.

EGA version is the best.
Rewatching The Grudge (Director's Cut), and it's still a very well made ultra creepy movie. A good remake too, pretty much on the same level with the original. American version has a bonus - a beautiful score by Christopher Young.
